HRA Widget

The Health Risk Assessment (HRA) Widget will appear in teal within the dashboard and can be used to access and view your Health Risk Assessment results or to start a new assessment. If an assessment has been completed, you will see your current health score on this widget. 

HRA Dashboard

Click on View My Health Score within the HRA Widget to view your access the HRA Dashboard. Here, you can view the results of past assessments, see if the HRA is a required activities, start a new assessment, or download your reports. 

The HRA Dashboard will show the last health risk assessment score. Click on Start New Assessment to start a new assessment.

Below the option, you will be able to see if the HRA is a required activity. If it is required and has not been completed, you will see an X in the Completion Status Column. Once the HRA has been completed, a Checkmark will be displayed.

Taking an Assessment

To start a new Health Risk Assessment (HRA), click on Start New Assessment. You will be redirected to a screen where the assessment can be started. Answer each assessment question by clicking on the appropriate option. 

Once all questions have been answered, click Finish. Your HRA results will be updated accordingly and can be viewed at the bottom of the dashboard for the completed Assessment Date.

Assessment History

Once an assessment has been completed, your information will be shown at the bottom of the HRA Dashboard. For each completed assessment, you will see the Assessment Date, Health Score, PDF Report Download Icons, and the Status.

The Assessment Date is the actual date the assessment was completed. For example, let's say you started the assessment one day, but then finished it two days later. The assessment date will be the date it was completed and not the day it was started. 

To download the Physicians Report or your Personal Report, click on the PDF Download option within the appropriate column for the Assessment Date.
