Forms Tab Purpose

The purpose of the Forms Tab is view forms for the selected user. Depending on the access permissions, you may be able to see only the form status or the form details. 

Forms Tab Overview

The Forms Tab can be used to view the details of uploaded forms for the user or see the status of a form. The form could be anything such as an physician form, an explanation of benefits, or a health club reimbursement, etc.

What is displayed on this tab will depend on your access level (i.e. user role). If you are an admin, you will only be able to see the status of the form. Depending on permissions, you may be able to see the form details. 

Important: An admin can only view the user's PII (Personal Identification Information) and Status data in the system. They cannot see PHI (Personal Health Information) in the system for a user. 

The Forms Tab will not be available until the user has registered (created an account) in the system. Please remember that a user cannot register until you have added their eligibility data.

Accessing the Forms Tab

To access the Forms Tab, click on Admin > User Search. Then search for the user and click on the Pencil Icon to manage their information. Once there, click on the Forms Tab.
