Error Code Purpose
The purpose of the Eligibility File Error Codes is to understand the meaning of the code and how to fix the data using FitLyfe admin interface or Client Human Resource Management System.
Overview
The following possible error code that can be triggered after processing the eligibility file:

Same member with different SSN | An eligible member with the same unique id, date of birth, and employee ID exists in FitLyfe but the SSN has changed. |
The steps must be done prior to importing the new eligibility file |
Age less than 18 | The age of the individual member is less than 18 | Update the individual date of birth of the impacted member in your system first so they can be qualified for FitLyfe program. The steps must be done prior to importing the new eligibility file |
Associates with mismatching Employee ID | The existing employee id in FitLyfe is different than the employee id in eligibility file |
The steps must be done prior to importing the new eligibility file |
Null SSN | SSN is a required field | Ensure that SSN is populate for each member in eligibility file |
InPlan is missing | In plan (Part of the health plan indicator) is a required field | Ensure that InPlan is populate for each member in eligibility file |
Two (or more) records contain the same SSN | Two different members using the same SSN | SSN must be unique to each member. It need to be fixed at the source |
Missing or incorrect location | Location code is missing or does not match with FitLyfe |

Unique Id is missing | Unique id is a required field | Ensure that every member has a unique id and must be unique within the system |
InPlan is missing | InPlan is null or empty for the given record | InPlan is a required field Acceptable values are 0 or 1 |
Important information
- Changes to the unique Id will result in preventing the user from accessing Wellsource Health Risk Assessment. If you have to changed the Unique Id, then the change must start in FitLyfe using admin section first then login to WellSource and update the username for the intended user.
- When an account is terminated, it can be re-activated after three months from the termination data.
