You must be logged into the system as a Coach or Coach Manager to have access to this information.

Screening History Purpose

The purpose of the Screening History is to add and manage the member's biometric screening data and to complete electronic consent for on-site screenings.

Screening History Overview

The Screening History Tab can be accessed by clicking on Coach Center > Coach User Search. After locating the member, click on the Pencil Icon and then select the Screening History Tab

Here, you can view the screening history for the individual, add a new screening, or edit an existing screening for the member.

Depending on what information has been entered for a screening, you will see the results within the table for each screening along with the Screening Date and the Screening Type.  

The Screening History for a member defaults to whatever date range is shown on this tab. Use the Service Date From and To fields to change enter a specific date or to configure a date range for viewing the account history.

The values displayed for each result of a screening within the table are color-coded based on the National Guidelines. These flags are for informational purposes only.

  • Green - The value falls within the normal range based on the national guidelines.
  • Red - The value is high compared to the national guidelines. This is a flag to indicates high risk. 

Note: If something is flagged high risk in the screening history, the member should contact their physician for further instructions and assistance. 

Depending on the number of screenings displayed for the configured date or data range, you will be able to use the page options at the bottom to view additional history items or to maneuver back and forth between pages.

Downloading a Screening Report

To download a screening report, click on the Screening Date or the Report Icon within the Screening History Tab.

The PDF file will be downloaded to your computer. In this report, you  will see the name of each test, the test result, and the normal value for each test. 

Adding a New Screening

Keep in mind that the Service Date is the only required field on the screening form. All other information is optional and does not have to be entered to save the screening. 

Depending on the design of the program, you may or may not have risk factors on this form. This information will not appear on the form unless you have contacted a FitLyfe Account Manager.

Keep in mind that it is dynamic meaning that information displayed here will depend on the configuration. Consult with your FitLyfe Account Manager about including risk factors in the program design. 

To add a Biometric Screening, complete the following steps:

Step 1: Click on Add New.

Step 2: Only the Service Date is required on this form. You do not have to complete every field to submit the screening; however, some of these fields might be required, depending on the incentive plan.

As data is entered, the BMI will be calculated automatically. The data will be verified from the standard ranges for each item. If the data is out of this range, the system will not be accept it. Some fields accept decimals and others do not on this form. 

It is important that when you enter the data, every field has acceptable ranges with the exception of certain fields capable of accepting NA (Not Applicable) or Greater Than and Less Than (<>)

  • Tri-Glycerides - Will accept Less Than (<) or Greater Than (>)
  • LDL Cholesterol - Will accept NA.
  • Total Cholesterol - Will accept Less Than (<) or Greater Than (>).
  • HDL Cholesterol - Will accept Less Than (<) or Greater Than (>).

For example, the Total Cholesterol can be set to <100 and the LDL set to NA.  Another example might be setting Tri-Glyderides to <45 and the LDL set to NA.

Step 3: Additional options may appear as options are selected on this form. For example, if Tobacco Use is selected, then you will see other options for selecting the type of tobacco use: smokeless, cigarettes, cigars, etc. 

Step 4: The Pregnant option will also be present for any female member asking if they are pregnant or not. 

Step 5: The Screening Type menu may be used to choose the type of screening data being entered. This data may be obtained on-site or from a submitted physician form. 

Step 6:  Flu Vaccination information may be entered for the member. Check the option to indicate the member received their flu shot and then use the Service Date field to the right of this option to enter the date they received their flu shot. 

Step 7: If Send Email to User is checked, the member will receive an email after the data has been entered for the screening. The user will be notified that their screening results have been received. If there are any incentives, then the incentives will be calculated.

Step 8: The Consent Status means the member has authorized to withdraw blood from them and then calculate their incentive, depending on the consent. The status will show "Not Received" initially on the form. 

The paperwork is frequently completed on-site for the screening including receiving electronic consent from the member. When the screening is done on-site, you will check the Consent Received checkbox. 

When this option is checked, a pop-up electronic consent form will appear on the screen. This form will have verbiage that must be read to the member on-site, who then must repeat the verbiage to provide consent.

Once this has been finished, the member will need to submit their PIN code. After the code has been entered, click Submit to complete on-site electronic consent and close the form. The Consent Status will now show Received in green.

This information can be found by the user upon logging into the system and viewing their account information. The security PIN code is on Security Tab. For more information about the member PIN code, please see the User Account Information page of the User Guide.

Step 9: Click Save. After the record has been saved, auditing information will be present at the bottom of the form any time it is opened for updates.

This shows when the screening was created and who created it. It will also show when it was last updated and who made the update. 

Updating a Biometric Screening

Any time updates are made to biometric data, FitLyfe keeps a historical record of the data for auditing purposes. The last update date and person who completed the update are displayed at the bottom of the screening data form. 

To make changes to screening data, click on the Pencil Icon for the appropriate screening within the history. 

Once all changes have been made, click Save to finish. 


Troubleshooting FAQ

How do you process electronic consent? 

You will need to check Consent Received on the screening form.

Then read the verbiage on the pop-up and have the member repeat.

Next, as for or have the member enter their security PIN code and click Submit.
